The BIR advised the public of the Lost Twenty-Eight (28) Sets of Unused/Unissued BIR Form No. 0535 – Taxpayer Information Sheet.

The lost sets of unused/unissued BIR Form No. 0535 – Taxpayer Information Sheet bears the following serial numbers:

| TIS202000141601 – TIS202000141615 | 15 sets |
| TIS202000141620 – TIS202000141625 | 6 sets |
According to BIR, the abovementioned forms were reported as lost by Mr. Asgar C. Macabanding, Revenue Officer II (A), Revenue District Office No. 41, Revenue Region No. 7B, East NCR, and have consequently been cancelled.
BIR emphasized that all official transactions involving the use of said forms are therefore considered as INVALID.
